💉 Also required: Certificate of Immunization Status (CIS)

Washington State requires a completed CIS on file before your child’s first day. This one is not ours to give you, so please send it with your other forms, by email or on paper.

Three ways to get it:

  • Print it yourself from MyIR, the state immunization registry. Your child’s record fills in automatically, and no signature is needed. (How to sign up)
  • Ask your child’s doctor to print it from the Washington Immunization Information System.
  • Contact the Department of Health at waiisrecords@doh.wa.gov or 1-866-397-0337.

If you would rather fill in the blank form by hand, you can download it from the Department of Health (form DOH 348-013).

Not sure which shots your child needs? The Department of Health publishes the official list for child care, and it is the one we go by:

If you are claiming a medical, religious or personal exemption, send us a completed Certificate of Exemption instead of the CIS. A child who is partway through a vaccine series can still start, as long as the CIS shows the catch-up schedule they are following.

Heads up: a printout from MyIR or from your doctor is ready to use as it is. If you fill in the blank form by hand instead, we can only accept it if your child’s health care provider signs it, or you attach a copy of their immunization records.

📝 How to Submit Your Forms

You have two options to submit your completed enrollment forms:

  • Option 1: Complete the forms on your computer and email them to Info@NurtureWithCareKidsAcademy.com
  • Option 2: Print the forms, complete them by hand, and bring them to the center

💳 Making a Payment

You can pay your $100 registration fee, your two weeks of advance tuition, ongoing tuition, or drop-in care by cash, check, Venmo or Zelle on our payment page. Once enrolled, you can also pay tuition by direct ACH in our parent app. Drop-in care is cash, Venmo or Zelle only. Please include your child’s first and last name and what the payment is for, so we can match it to your family.

Families using DSHS, MCCYN, or MCCYN-PLUS should talk to the Director first rather than paying here.
